coughing and sneezing and waking up 5am

I think my husband is in the middle stage with Vascular Dementia, he has been getting more symptoms recently. He chokes and coughs when he has drinks and sneezes when he eats which is hard to watch. He has started waking and getting dressed at 5am, he usually wakes from his afternoon sleep 5pm. He is also hallucinating and having vivid dreams. He gets muddled with our son and his brother, but can seem ok when our son visits ,so they don't see what he is like when we are home alone. I can only go out when he visits but obviously I want to see our son aswell. We had a carers assesment over the phone which I wasn't expecting, it is very difficult to say what you feel over the phone to some one you don't know and I haven't heard anything more
